JMS Registration Information 2011-12

New students to Jefferson City Schools should complete the registration forms below and bring them with you. Please plan to provide the following documents when registering your child:

Proof of Residence (water bill, power bill, etc.)

Birth Certificate

Immunization

Social Security Card or sign a waiver to decline

CRCT and other test scores if available

Compulsory Student Attendance Law

Home Language Survey

Out of District students only - Student and Parent Contracts for Out of District Students

All students entering sixth grade must meet the following requirement:
Two doses of Varicella (chicken pox) vaccine or documentation of disease or laboratory proof of immunity.
